operating a healthcare facility under the name Nido Family Centre (Rodzinne Centrum Nido)
§1. General Provisions
- These Organisational Regulations, hereinafter referred to as the "Regulations", set out the manner and conditions under which healthcare services are provided by the healthcare entity operating under the business name Nido Marta Karwot-Pięta Usługi Neurologopedyczne, hereinafter referred to as the "Entity", which operates a healthcare facility under the name Nido Family Centre, hereinafter referred to as the "Centre".
- The Entity is registered in the Central Register and Information on Economic Activity (CEIDG) and in the Register of Entities Performing Medical Activities maintained by the Voivode of Małopolska under registry number 000000276874.
- Entity identification data: NIP (Tax ID): 6472186653, REGON: 365831356, healthcare facility REGON: 36583135600029.
- Registered office and place of healthcare provision: ul. Władysława Żeleńskiego 86, 31-353 Kraków, Poland.
- These Regulations have been established by the Head of the Entity pursuant to Articles 23 and 24 of the Act of 15 April 2011 on Medical Activity (Polish healthcare legislation).
§2. Objectives and Tasks of the Entity
The objective of the Entity is to provide healthcare services focused on comprehensive, interdisciplinary diagnosis and therapy of developmental, neurological speech therapy, and speech and language disorders, as well as physical rehabilitation for children and adults, with particular emphasis on infants, children with feeding disorders and neurodevelopmental disorders, as well as health promotion and the propagation of healthy behaviours.
The tasks of the Entity include in particular:
- provision of healthcare services in the fields of neurological speech therapy, speech and language therapy, feeding therapy, physiotherapy, medical rehabilitation, osteopathy, sensory integration therapy, clinical psychology, child and adolescent psychiatry, orthopaedics, ultrasonography, frenectomy, ADOS-2 diagnostic assessment, and therapeutic massage;
- ensuring high quality of healthcare services in accordance with current medical knowledge and applicable standards;
- implementation of an interdisciplinary care model;
- maintaining medical records in accordance with applicable regulations;
- upholding patient rights;
- undertaking activities in the field of health prevention and education.
§3. Type of Medical Activity and Scope of Healthcare Services
The Entity conducts medical activity in the category of outpatient healthcare services. All healthcare services are provided on a fee-for-service basis only. The Entity does not hold a contract for the provision of healthcare services funded from public resources (NFZ — National Health Fund).
The scope of healthcare services provided includes:
- neurological speech therapy and speech and language therapy — diagnosis and therapy of speech and communication disorders, including those of neurological origin;
- feeding therapy — interdisciplinary diagnosis and therapy of feeding disorders in infants and children (NIDO Eat concept);
- physiotherapy and medical rehabilitation — functional assessment, kinesitherapy, physical rehabilitation for children and adults, including NDT-Bobath therapy;
- osteopathy — diagnosis and manual therapy for musculoskeletal, visceral, and craniosacral dysfunction;
- sensory integration therapy — diagnosis and therapy of sensory processing disorders;
- clinical psychology — psychological assessment, consultations, and psychological support;
- child and adolescent psychiatry — psychiatric consultations;
- orthopaedics and musculoskeletal trauma — orthopaedic consultations, musculoskeletal assessment;
- paediatrics — paediatric consultations;
- ultrasonography (ultrasound) — ultrasound examinations;
- frenectomy — diagnosis and surgical treatment of restricted frenula;
- ADOS-2 diagnostic assessment — standardised assessment for autism spectrum disorder;
- therapeutic massage — massage performed for therapeutic purposes;
- lactation counselling — breastfeeding support.

§4. Place of Healthcare Provision
Healthcare services are provided at the Centre's premises at: ul. Władysława Żeleńskiego 86, 31-353 Kraków, Poland. The Entity does not provide healthcare services via telecommunications systems (telehealth consultations).
§5. Organisational Structure of the Healthcare Facility
The Entity operates one healthcare facility under the name Nido Family Centre (REGON: 36583135600029).
The following organisational units operate within the healthcare facility:
- Ultrasound Unit — performing ultrasound examinations;
- Orthopaedic Clinic — orthopaedic consultations, musculoskeletal assessment;
- Physiotherapy — physiotherapy, medical rehabilitation, osteopathy, sensory integration therapy, therapeutic massage;
- Neurological Speech Therapy — neurological speech therapy, speech and language therapy, feeding therapy, ADOS-2 assessment, frenectomy;
- Paediatric Clinic — paediatric consultations, lactation counselling;
- Child Mental Health Clinic — clinical psychology, child and adolescent psychiatry.
§7. Process of Healthcare Provision
Healthcare services are provided exclusively by individuals holding the appropriate professional qualifications.
Booking an Appointment
Patients may book an appointment in person, by telephone on +48 577 550 025, by email at [email protected], or via the online booking system available at www.nidomed.pl.
Cancellation Policy
If a patient is unable to attend a scheduled appointment, they should notify the Centre as early as possible, no later than 24 hours before the scheduled appointment time, in order to make the slot available to another patient.
§8. Patient Rights
The Entity upholds patient rights, and in particular:
- The patient has the right to healthcare services that meet the requirements of current medical knowledge.
- The patient has the right to information about their health condition, diagnosis, proposed diagnostic and treatment methods — in an accessible and comprehensible manner.
- The patient has the right to give consent to specific healthcare services or to refuse such consent.
- The patient has the right to have their privacy and dignity respected during the provision of healthcare services.
- The patient has the right to access their medical records.
Healthcare services for minor patients are provided with the consent of their legal guardian.
§9. Payment for Healthcare Services
All healthcare services provided at the Centre are subject to a fee.
Payment for healthcare services may be made by:
- cash,
- payment card,
- bank transfer.
For each healthcare service provided, the patient receives a fiscal receipt or — upon request — a VAT invoice.
§10. Access to Medical Records
Medical records are made available to the patient, their legal guardian, or a person authorised by the patient — upon presentation of a valid identity document.
The first disclosure of medical records in the requested scope is provided free of charge.
For each subsequent disclosure, the Centre may charge a fee:

§12. Personal Data Protection
The data controller for patients' personal data is the Entity: Nido Marta Karwot-Pięta Usługi Neurologopedyczne, ul. Władysława Żeleńskiego 86, 31-353 Kraków, Poland, NIP: 6472186653. Contact details for personal data protection matters: [email protected].
§14. House Rules
The following are strictly prohibited on the Centre's premises:
- smoking tobacco products and using electronic cigarettes,
- consuming alcoholic beverages,
- taking narcotic drugs or psychotropic substances.
Patients are required to arrive punctually for their scheduled appointments. In the event of a significant delay, it may not be possible to provide the service in full.
§15. Emergency Procedures
In the event of a medical emergency occurring on the Centre's premises, staff are obliged to provide immediate first aid and to call the emergency medical services (emergency number: 112 or 999).
§17. Complaints and Suggestions
A patient or their legal guardian has the right to submit a complaint or suggestion regarding the healthcare services provided or the operation of the Centre.
Complaints and suggestions may be submitted:
- in writing — to the Entity's registered address: ul. Władysława Żeleńskiego 86, 31-353 Kraków, Poland,
- by email to: [email protected],
- in person — at the Centre's reception.
Complaints are reviewed by the Head of the Entity within 30 days of receipt.
Independently of the option to submit a complaint to the Head of the Entity, the patient has the right to contact:
- the Patient Rights Ombudsman (Patient Information Helpline: 800 190 590),
- the Voivode of Małopolska as the authority maintaining the register of entities performing medical activities.
§18. CCTV Monitoring
The Centre may use video surveillance in publicly accessible areas (reception, corridors, entrance) in order to ensure the safety of patients and staff and to protect property. Monitoring does not cover rooms where healthcare services are provided, sanitary facilities, or any other areas where recording images could violate the dignity or privacy of a patient.
§19. Final Provisions
These Regulations come into force on the date determined by the Head of the Entity. The current text of the Regulations is made available to patients by displaying it in a visible location at the Centre and by publishing it on the website www.nidomed.pl.
